It is very important that you notify the Practice if you have moved home. We require your current address details to be on your patient record so that if needed we may contact you regarding your personal health, for referral purposes and should the NHS wish to contact you.
We can only register patients in our catchment area (see our Registration page for a list of these postcodes). Once registered with us, if you move outwith our catchment area you must register with a Doctor who covers that catchment area as soon as possible. If you fail to do so the Practice will remove you from our patient list.
We cannot change your address on our record to one outwith our catchment area as we do not provide care outwith our specified catchment area. Our local Health Board routinely asks GP Practices to check patient addresses to establish if they are still currently registered at the address provided by the patient if there is a number of returned mail items such as vaccination invites. If your GP Practice is unable to confirm your details, then the Health Board may remove you from the Practice List.
You can telephone your change of address to the staff on 0141 483 1740, in person at the surgery, or complete the “update your personal details form” on our website*. We will then update your medical records accordingly. If outwith our catchment we will inform you and give you notice of when you will be removed from our patient list.
If you move outwith our catchment area on a temporary basis (less than 3 months) you can register with a local GP as a Temporary Patient, they can request a medical summary from us so they can provide care if required. If you are still in temporary accomodation and outwith our catchment area after 3mths you will be notified by letter that you are being removed from our patient list. Your records will be transferred to NHS GG&C and once you register as a full time patient at another Doctor your records will then be transferred to them. If you return to living on a full time basis within our catchment area you can reapply to register with us.
*Your personal information provided, will be solely used to process your change of address request. It will not be passed to any third party, without your consent, unless required by law.This conforms to the Data Protection Act of 1998.
Please note that if you move outwith our catchment area for registrations, you will be removed from our Practice List and informed to register with a local GP, there are no exceptions to this as catchment areas are there to ensure GP surgeries have a manageable patient list size and within suitable distances to provide home visits should they be required.
